1. Select Bankruptcy menu.
2. Select Motions/Applications.
3. Enter case number.
4. Ensure correct case name and number match the document you are filing.
5. Select Docket Event: Dismiss Chapter 11.
6. Check box if joint filing with another attorney.
7. Select party filer.
Party filer not listed, Add/Create New Party.
8. Browse, verify and attach the document (PDF file).
Attach supportive documents, if applicable.
9. Is this for failure to make plan payments? Select Yes or No.

10. If you did not select Yes or No a warning message will appear: "You must make a selection on the preceding page-click on browser back button to correct."
If Action Canceled or Page Expired appears when you click Back button Start Over!!
11. Verify the final docket text before submitting onto case docket.
12. Review the Notice of Electronic Filing. It is your receipt.
