Employee Income Record

Effective Date:  May 8, 2014    Supersedes:  August 25, 2009

Note:  Use this docket event for Amended Employee Income Record.

Attach copies of all payment advices or other evidence of payment received within 60 days of filing date for all employers.

If the debtor does not have 60 days of records, file a W2 with redacted SSN and affidavit or declaration in lieu of lost wage statement.

Use the docket event Declaration Re: No Employee Income Record when Employee Income Record is not relevant:

Keystrokes

1.  Select Bankruptcy menu.

2.  Select Other/Misc.

3.  Enter Case Number.

4.  Ensure correct case name and number match the document you are filing.

5.  Select Docket Event:  Employee Income Record.

6.  Check box if joint filing with another attorney.

7.  Select party filer.

8.  Display message:  BEFORE attaching the document(s), please review to ensure that personal identifiers including the Social Security Number have been redacted.

9.  Browse, verify and attach the document (PDF file).

10. Select "Amended" from the drop down list if Amended Employee Income Record.

11. Verify final docket text before submitting onto case docket.

12.  Review the Notice of Electronic Filing.  It is your receipt.
