Use this event to docket Appointment of Ombudsman after Order on Motion to Appoint Ombudsman is granted.
Note: Certificate of Service required.
1. Select Bankruptcy menu.
2. Select Trustee/US Trustee.
3. Enter case number.
4. Ensure correct case name and number match the document you are filing.
5. Select Docket Event: Appointment of Ombudsman.
6. Select party filer from pick list.
Pick [Trustee name].
7. Browse, verify and attach the document (PDF file).
Attach supportive documents, if applicable.
8. Search for a Party screen.
Enter social security number, tax identification number, last, first and middle name or business name to search for Ombudsman in the CM/ECF database.
Party Search results:
Ombudsman name appears: Select the name from the list.
Ombudsman name does not appear: Select Create New Party.
Select Party Role as Health Care Ombudsman or Consumer Privacy Ombudsman.
Verify once more for correct party information and Submit.
9. Enter the role of the ombudsman: Consumer Privacy or Health Care. Entry is required.
10. Enter the name of the ombudsman. Entry is required.
11. Verify final docket text before submitting onto case docket.
12. Review the Notice of Electronic Filing. It is your receipt.
